Context:
The Health Care Collaborative of Lafayette County (HCC Network) underwent tremendous growth in recent years, evolving from a rural health coalition into a Federally Qualified Health Center operating multiple clinic locations. This exponential growth brought new funding and patients but also presented challenges.
Challenge:
With this scale and complexity, HCC needed a strategic planning process to support alignment across its expanded organization. Their prior plan was dense and difficult to implement day-to-day. The new plan had to shed outdated strategies, delineate board and staff roles, and ultimately produce a simple, focused roadmap accessible to all staff.
Solution:
